Yes, there's an MTX shop
Diablo 4 will have a microtransactions shop [1] (or MTX), where players can purchase cosmetic-only items for their characters with real money:

Past games with a "pay-to-win" or "P2W" approach (i.e., you can purchase features or items that make your character more powerful) have alienated players in the past. Much like Path of Exile has done, Diablo 4's microtransactions will stick to cosmetics. This was confirmed in June 2022 [2] by Diablo's Community Manager, Adam Fletcher:

Also from Diablo's Community Manager, Adam "PezRadar" Fletcher, we know that Diablo Immortal players can use Blizzard's Battle.net Balance to pay for microtransactions [4]:
Although not confirmed, we expect this to be true for Diablo 4's in-game cash shop as well. Players of other Blizzard games, like Overwatch and World of Warcraft, likely have existing balances in their account that can conveniently be applied to Diablo 4 items; plus, they already know how to use this form of payment.
