With Diablo 4, Blizzard has repeatedly said it's drawing a lot of inspiration from Diablo 2. Diablo 2 introduced the Secret Cow Level, sometimes referred to as "Moo Moo Farm" and known as one of the most famous hidden levels in all of gaming. Given how Blizzard wants to recreate much of the magic Diablo 2 had, it wouldn't be surprising to see a Cow Level in Diablo 4, especially since Diablo 3 didn't launch with one (it'd come later in a patch and be called "Not The Cow Level").
TOPDiablo 2's Cow Level
In all versions of Diablo 2 (base game, Lord of Destruction expansion, or Resurrected), players can access the Cow Level by combining a Town Portal Tome and Wirt's Leg in the Horadric Cube. This will open a portal, which leads to a small field where hordes of cows carrying polearms enter the screen and target the player. All of the cows are the same, with the exception of the Cow King, which has a few affixes, like Lightning Enchanted and Magic Resistance. Given the density of cows and how easy they are to kill, the Cow Level was once a great place to farm items -- until it was nerfed.
TOPDiablo 3's Not The Cow Level
Diablo 3 launched with a secret level called Whimsyshire, a forest filled with rainbows, stuffed bears, and unicorns. Blizzard, seeing that players missed their polearm-carrying cows, introduced "Not The Cow Level" (NTCL) for Diablo 3's third anniversary. The level can be reached by transmuting a Bovine Bardiche in Kanai's Cube, which opens a portal. This cow level has a variety of cow monsters, including ones wielding spears and casting spells as shamans, some of which can be elite level. Lastly, the cows are led by the Cow Queen and the Ghost of the Cow King.
